SMS Health Center Offers Free Sports Physicals

The Clarkston Community Schools Health Center at SMS (operated and funded by the Clarkston Medical Group)
with offer free sports physicals for all current fifth through seventh grade students in the district as part of its mission to provide high quality, evidenced-based care to enhance students' overall well-being and academic success, while eliminating barriers to obtaining healthcare services.

The Clarkston Community Schools Health Center at Sashabaw Middle School is offering FREE sports physicals for all Clarkston Schools students who are currently in 5th-7th grade and their siblings, as well as all NSE students (K-5) and their siblings. The walk-in clinic will be held Thursday, June 7 from 12pm-4pm at SMS, and students must be accompanied by a parent or guardian.

Please download and complete the MHSAA Sports Physical & Medical Health History Form, and bring it with you on June 7.

Cost

Sports physicals are free of charge for all current and incoming SMS students.
