Change of Address

1. Student Change Notice - Download and complete  the Student Change Notice Form to change your and/or your student's address. 
2. Proof of Residency - Must provide one document from the "Proof of Ownership or Occupancy" tab and two supporting documents from the "Supporting Documents" tab. 
3. Return your Student Change Notice Form and Proof of Residency documentation to Central Enrollment. Until the Central Enrollment resumes normal office hours, please email documents to . Please do not come to  the Enrollment Office.
