Student Health

At Clarkston Community Schools, we prioritize student health and safety. For students with health and medical needs requiring medications or interventions during school hours, it’s essential to have the appropriate documentation in place.

To ensure our staff is prepared to provide necessary services, we require updated Medication Administration Authorization (MAA) Forms and Medical Plans of Care (POCs) at the beginning of each school year. These forms must be completed annually and will expire on the last day of instruction. They provide critical medical direction from your child’s healthcare provider, allowing our staff to administer medications and perform medical interventions as needed.

If you have any questions or need assistance with the forms, please reach out to the school office or district nurse. Thank you for your cooperation in helping us support your child's health at school!
