You may obtain the Registration Forms one of two ways:  
1. From the Early Childhood Center
2. From our website

Either way you MUST bring them to the Early Childhood Center for processing and payment.  There is a Registration Fee to be sumbitted with your Registration Forms.  We DO NOT accept Registration Forms via the Internet.

You may print the Registration Forms by clicking on the Link below.  However, you MUST:

1.  Fill in EVERY line - our forms are audited by the State of MI Child Daycare Licensing Division and we will NOT accept incomplete forms.

2.  Read the Parent Handbook online.  You will be asked to sign that you have read it and agree to abide by the policies as outlined in the Parent Handbook.

3.  Bring your completed Registration Forms (3 pages) to the Early Childhood Center.
